Nominate a Nurse for a DAISY Award
The DAISY (Diseases Attacking the Immune SYstem) Award for extraordinary nurses, honors and celebrates the skill, compassion, and care that nurses provide to their patients every day!
The DAISY Award was established by the DAISY Foundation in memory of J. Patrick Barnes who died at 33 of ITP, an auto-immune disease. The Barnes Family was awestruck by the clinical skills, caring, and compassion of the nurses who cared for Patrick, so they created this national award to say thank you to nurses everywhere.
You may nominate a nurse for The DAISY Award any time throughout the year. Just fill out and submit the form below OR pick up a nomination form at one of our health centers then drop it in a nomination box, hand it to one of our staff, or mail it in.
